A member can receive a 5% discount on additional services and products that the company Healthy Improvements offers.
If you would like to split the massage membership with your spouse / child / or family member (example: brother/sister/father /mother) you can do so by filling out and signing the additional persons section of this agreement. You may not have more than 3 people per plan.
Please make sure to let us know if you have any changes in contact information, phone number, address, email address or credit card information.
When a member is running late, the massage session will be shortened by the amount of time they are late up to the full 30 minutes. If member arrives over 30 minutes late or does not show, then they have forfeited the monthly massage, or will be charged a fee.
Sign up membership fee of $25 per person is non-refundable.
